🎬Watch How It Works
Not sure where to start? This short video walks through the whole flow: pick a gender, fill in a start date, generate the data and the sheets, then edit every sheet to match your real headaches before printing or saving as PDF.
Always recorded as YYYY-MM-DD (e.g. 2016-11-06). The picker follows your browser's language and may read mm/dd/yyyy — the stored value is identical.
💌1. Generate Data
Generated entirely on this machine — no external API. Uses gender, start date and end date (start + 89 days); the result is kept as JSON and printed with console.log (F12). Each run differs.
📒2. Build Sheets
What is generated is sample data, not your actual condition. Once the sheets appear, edit them cell by cell to match your real headaches: click a pain cell to cycle 3 / 2 / 1 / blank, click a tick cell to toggle it, click the “headache duration” row to add an hour, and click a date to circle your period days. The diary you hand to your doctor must be a truthful record.
Every month is shown at once — one sheet per month.
You can still edit a sheet after it is built: click a pain cell to cycle 3 → 2 → 1 → blank (hold Shift to go backwards), click a tick cell to toggle it, click a cell in the duration row to cycle blank → 1 → … → 8 hours (hold Shift to go backwards), and click a date to circle or un-circle a period day. Print once you are happy with it.
🖨️3. Print / Save as PDF
Prints every month in one job, one page each (A4 landscape). Choose “Save as PDF” as the destination.
💊Emgality (Galcanezumab) in Taiwan
Emgality (galcanezumab) is a monoclonal antibody targeting CGRP (calcitonin gene-related peptide), approved for the preventive treatment of migraine in adults. The notes below are compiled from the Taiwan FDA (TFDA) package insert, Taiwan Headache Society guidelines and National Health Insurance (NHI) regulations, for reference only.
Dosage
Form: Subcutaneous injection, as a prefilled pen or syringe, 120 mg per unit.
Loading dose: Two consecutive 120 mg injections (240 mg total) at the first dose.
Maintenance: One 120 mg injection every month thereafter.
Missed dose: Inject as soon as possible, then restart the monthly schedule from that date.
Self-injection and storage
Warm up: Take it out of the fridge (2–8 °C) and let it sit at room temperature for 30 minutes; never microwave, heat or shake it.
Clean: Wash your hands, disinfect the injection site with an alcohol swab and let it air-dry.
Site: Abdomen (5 cm away from the navel), thigh, back of the upper arm or buttocks; avoid bruised, red or hardened skin.
Inject: Press flat against the skin to unlock, then press the button; the first click means it started, the second click (about 10 seconds) means it is done.
Storage: Keep refrigerated and protected from light; it may stay below 30 °C at room temperature for up to 7 days, but must not go back into the fridge once warmed.
Taiwan NHI reimbursement criteria
Emgality costs roughly NT$10,000–20,000 per dose out of pocket. NHI coverage requires prior authorization and is limited to refractory chronic migraine, under the following criteria:
Criterion
Requirement
Prescriber
Diagnosed and prescribed by a board-certified neurologist or neurosurgeon.
Administration
Must be injected at a medical facility by staff; take-home doses are not reimbursed.
Diagnosis
Chronic migraine: for 3+ consecutive months, headache on ≥ 15 days per month, of which ≥ 8 days show migraine features.
Prior treatment
Failure of, or intolerance to, at least 3 conventional oral preventives (e.g. topiramate, beta-blockers, calcium-channel blockers).
Evidence
A complete, truthful headache diary covering at least 3 months must be submitted as the core clinical evidence.
Duration
First approval covers 3 months; a ≥ 50% reduction in headache days earns another 3 months (max 6 months per year), followed by a mandatory 6-month break before reapplying.
The headache diary is exactly the document format this site's sheet generator helps you prepare.
Side effects and precautions
Common: Injection-site pain, redness, itching and local inflammation (about 10%); some people get mild nasopharyngitis.
Serious allergy: Seek immediate medical care for breathing difficulty, severe hives or facial swelling after an injection.
Special populations: Not recommended during pregnancy or breastfeeding due to limited safety data; no dose adjustment for mild-to-moderate hepatic or renal impairment.
Before you see your doctor
If you are considering Emgality, check these three things before your appointment:
Have you kept a headache diary for at least 3 months?
Is your migraine episodic (fewer than 15 headache days per month) or chronic (15 or more)?
Which conventional oral migraine preventives have you tried, and how did they work out?
This information is for reference only and is not medical advice. Always follow the latest package insert, current NHI rules, and your doctor's and pharmacist's instructions.
